Dear Pharmacist, I am 90 pounds over-
weight and just began taking the medication
Xenical. Is there another drug that works better
than this? Are there any supplements you rec-
ommend?
Dallas, Texas Answer: Kudos to you
for getting on the path to wellness. This topic
could encompass an entire book
and if there was one easy
answer it would be
great but there’s not.
There isn’t a medica-
tion that 1 recommend
for weight loss, although
the orlistat (Xenical) is com-
monly prescribed to assist in weight loss, it is
not a fat burner. It is a fat blocker. It reduces
intestinal fat absorption by blocking an
enzyme called lipase. One way to prevent fat
in the first place is to keep your cortisol levels
down. Cortisol will rise in the presence of
chronic stress, fear and grief and the hormone
contributes to belly fat. You can evaluate corti-
sol and other adrenal hormones with urine and
saliva tests.
There’s probably no faster way to lose
weight than to just eat meats, veggies and
nuts/seeds and drink only water, tea or coffee.
Testing your thyroid is important because that
is often very low, despite normal blood tests.
Leptin resistance can cause you to hold on to
weight, and leptin can be measured via blood
tests too. How to test your thyroid and leptin is

mone, you can turn food into fuel.

•Tyrosine- An amino acid that is considered
a gentle appetite suppressant. It helps you
make thyroid hormone, which regulates
metabolism and bums fat. Turning calories
into energy is important. Unbumed calories
equals stored fat on you. Tyrosine helps you
make dopamine, a happy neurotransmitter that
influences appetite.
You often see this compound in thyroid sup-
plements and diet aids.
• Phenylalanine- An amino acid that helps
you make tyrosine (see above) while also trig-
gering the release of CCK (cholecystokinin), a
con und that I !ps you feel full. It’s what we
call a satiety hormone. People often take this
amino acid before meals. If you take tyrosine,
you would not need this and visa versa. People
with phenylketonuria should never take this
supplement.
• Avocado extract- It comes as a capsule,
tablet or powder. It's a gentle appetite suppres-
sant and people say it reduces carb cravings. It
should improve your glutathione status.
Glutathione is a major liver antioxidant that
you want more of.
• Cumin- A spice you can buy at any store
that some tout as a natural appetite suppressant
activity. Sprinkle it on food.
• Peppermint tea- Drinking peppermint tea
with your meal could help you reduce appetite,
but be careful it may exacerbate reflux.

FDA approves pacemaker,
defibrillator expansions
The Oklahoma Association
ot Optometric Physicians
(OAOP) is conducting a con-
test to find out how Doctors of
Optometry have changed the
lives of patients.
Comprehensive eye exami-
nations performed by opto-
metric physicians can detect
high blood pressure, diabetes,
risk of stroke, ocular cancer
and many other conditions
that may cause loss of sight or
compromise overall health. In
addition, their focus on the
relationship of vision to
school success, diagnosing
eye conditions in infants early
and helping those with low
vision concerns touch the lives
of Oklahomans even day.

The U.S. Food and Drug
Administration today
approved an application from
Medtronic for revised labeling
for two cardiac resynchroniza-
tion pacemakers (CRT-P) and
eight cardiac resynchroniza-
tion defibrillators (CRT-D),
expanding the indication for
use to patients with atrioven-
tricular (AV) block and less
severe heart failure.
About 5.1 million people in
the United States have heart
failure, a condition in which
the heart cannot pump enough
blood to meet the body's
needs. With proper treatment
and lifestyle changes, people
with heart failure can live
longer and more productive
lives.
The CRT-P and CRT-D
devices (see complete device
list below) work by providing
electrical impulses to the right
and left sides of the heart
through implanted leads to the
heart’s right ventricle (RV) and
left ventricle (LV). The timing
of the impulses is programmed
by a clinician who synchro-
nizes the patient’s heart to
improve cardiac function for
heart failure patients. The RV
leads used with CRT-D
devices are also capable of
delivering high voltage energy
to defibrillate the heart in the
event of ventricular arrhyth-
mia.
The FDA previously
approved these devices for
patients with more severe
heart failure as evaluated by
their physician using specific
criteria. The new approved use
includes patients with less
severe heart failure, but who
are already indicated to
receive RV pacing. With the
new indication, these patients
will be eligible to receive a
device that will pace both
sides of their heart instead of
just the right.
"Medtronic’s cardiac resyn-
chronization therapy pacemak-
ers and defibrillators can delay
the occurrence of heart failure-
related urgent care visits for
people who meet these new
criteria,” said Christy
Foreman, director of the
Office of Device Evaluation in
the FDA’s Center for Devices
and Radiological Health. "This
approval expands the indica-
tion of cardiac resynchroniza-
tion therapy to this additional
patient population."
The expanded approval for
the CRT-D and CDT-P devices
was based on data from the
Block HF clinical study that
compared death, heart failure-
related urgent care visits and
increases in left ventricular
end systolic volume index
(LVESVI) in study subjects
who received either left and
right ventricular pacing, or
right ventricular pacing alone.
LVESVI is a diagnostic meas-
ure of cardiac structure and
function. A total of 918 sub-
jects participated in the trial,
531 of whom received a CRT-
P device and 227 of whom
received a CRT-D device. The
study showed that the cardiac
resynchronization therapy
provided by CRT-P and CRT-
D devices results in a 27 per-
cent reduction in death, heart-
failure-related urgent-care vis-
its. and increases in LVESVI
compared to right ventricular
pacing alone.

Community Chat set for April 16
From the OSDH
As part of an effort to learn what
Oklahomans believe are their most crucial
health needs, the Oklahoma State Department
of Health (OSDH) is hosting a series of “com-
munity chats” throughout the state this month
through June. These meetings will focus on
what Oklahomans believe are the primary
health concerns of their communities. The feed-
back received will be used to update the
Oklahoma Health Improvement Plan, first
launched in 2009.
Tulsa will be the site for a Community Chat
on Wednesday, April 16, at the Community
Service Council, 16 East 16th St., from 5 p.m.
to 7 p.m. Residents are encouraged to drop by
and offer their input on how to strengthen those
community partnerships that will make a differ-
ence in the lives and health status of all
Oklahomans.
The Oklahoma Health Improvement Plan
was mandated by the Oklahoma Legislature in
2008 through SJR-41, which directed the State
Board of Health to prepare a report that outlines
a plan for the “general improvement of the
physical, social and mental well-being of all
people in Oklahoma through a high-functioning
public health system." The current version of
the Oklahoma Health Improvement Plan focus-
es on three targeted “flagship initiatives"
including children’s health improvement, tobac-
co use prevention, and obesity reduction.
