DENVER lUPI) - A surgi-
cal team that made medical
history by implanting a tiny
tube in the skull of an unborn
fetus must wait until the baby’s
birth next month to verify they
averted a near-certain defect.
The surgical implantation of
the shunting device to relieve
the fetus’ hydrocephalus —
pressure on the brain caused by
excess fluid — was revealed
Sunday by the Denver Post in a
copyright story. The procedure
had never before been success-
ful.
In the operation, the seven-
member surgical team installed
a custom-made silicone rubber
tube so small it could slide
easily through a hollow needle
into the left ventricle of the
fetus’ brain.
The pioneering surgery took
place April 29 at the University
of Colorado Health Sciences
Center, but was not publicly
revealed until Sunday. The Post
said it obtained a copy of an
article prepared by the surgical
team for future publication in
the New England Journal of
Medicine.
The unidentified mother has
given birth to a healthy girl but
has also had one other
hydrocephalic baby that died.

number of classes and training sessions in addition to working
toward merit badges in a variety of subjects. Taking part were

Although the doctors believe
they were successful in im-
planting the tiny shunt, they
won’t know if the procedure
succeeded in relieving pressure
on the brain until the baby is
born July 29 by Caesarean
section.
The mother, given a local
anesthetic, was conscious
during the operation.
"Correct placement of the
shunt was confirmed by ul-
trasouond,” the surgical team
wrote in the report. "The fetus
was active, fetal heart rate
remained stable and the patient
experienced only sporadic ute-
rine contractions."
The surgical team was
headed by Dr. William H.
Clewell, associate professor of
obstetrics and gynecology at
the CU School of Medicine.

The implanted shunt was
about 4 inches long and a
sixteenth of an inch wide. It is
held in place by small anchors
and is equipped with a valve to
limit the flow of fluid from the
brain and keep amniotic fluid
from flowing into the brain.
The newspaper said that at
two points during the surgery,
when uncertainty ended in
success, there were cheers.
"Under local anesthesia, a
small incision was made in the
maternal skin ... A blunt, thin-
walled 13-gauge needle with a
pointed stylet was inserted into
the amniotic cavity.
"The fetal head was stabil-
ized and the needle advanced
until point contact was made.
Steady pressure and a slight
twisting motion resulted in the
easy penetration of the fetal
skull ...
"Entry into the dilated
ventricle was visualized sono-
graphically and confirmed by
the free flow of clear cerebros-
pinal fluid from the needle. The
shunt was passed through the
needle and advanced into place
using a second blunt sylet."
The report said doctors
planned to replace the tempo-
rary shunt with another one
after birth. The second one, the
report said, would be complete-
ly covered by the baby’s skin
and would drain the fluid to the
abdomen.

Ten-year-old Johnnie had
trouble seeing the blackboard.
He did poorly at little league
baseball. He couldn’t focus on
the moving ball. An eye ex-
amination indicated that
Johnnie needed corrective
glasses for seeing at a distan-
ce. Johnnie came home with
Mom wearing his first pair of
glasses. As they entered the
front door, Johnnie’s father
was obvious about his dis-
appointment. This negative
attitude can be devastating to
the self-esteem of a youngster.
The greatest problems en-
countered with children who
wear glasses originate from
parents — not children. Child-
ren are quite willing and
sometimes even eager to wear
glasses. They are quick to
understand that glasses will
allow them better vision and
thus give them an easier time
at school and at play. The idea
that there is something wrong
is usually picked up from their
parents.
Advice: Try not to express
disappointment in your child’s
presence if glasses are indi-
cated. For the sake of the
child, try to be positive about
the visual benefits glasses
will afford.
